DYSFUNCTIONAL UTERINE BLEEDING: IS CLINICAL PRESENTATION AND EXAMINATION A RELIABLE GUIDE IN THE DIAGNOSIS OF THE HORMONAL DEFECT IN DUB?

ABSTRACT
Objective: There are various hormonal disturbances that can give rise to dysfunctional uterine bleeding. The objective of this study was to determine if underlying hormonal defect could be predicted from the clinical presentation.
Method: One hundred and thirty Pakistani women in whom initial clinical evaluation and ultrasound failed to find any obvious organic cause for abnormal uterine bleeding were enrolled in the study. An examination under anesthesia and diagnostic curettage was performed in all cases. All endometrial samples were subjected to histopathological examination and findings were correlated with clinical presentation.
Results: Out of 130 patients, histological diagnosis confirmed the clinical diagnosis in 101 patients. In 29 cases initial clinical diagnosis was found to be incorrect on histopathology.
Conclusion: Clinical presentation was proved to be a useful guide to underlying hormonal defect in the majority of cases of DUB. Endometrial biopsy can safely be deferred if initial thorough clinical assessment is performed. The reliability of clinical diagnosis in cases of abnormal uterine bleeding is significantly high. |
