NON SURGICAL MID TRIMESTER TERMINATION OF PREGNANCY BY MISOPROSTAL

ABSTRACT
Objective: To observe the efficacy of Misoprostal as a conservative method for mid trimester termination of pregnancy.
Design: Observational study Setting: Department of Obstetrics and Gynecology Unit II, Karachi Medical and Dental College. The study period was from December 2002 to December 2003.
Subjects: A total of 100 patients requiring mid-trimester termination of pregnancy for missed abortion, intrauterine death and structural anomaly between 14 and 20 weeks of gestation were included in the study. Each woman received 200 pg misoprostol vaginally. Six hours following the first dose of misoprostol 200 ug doses were administered to a maximum of 4 doses. Success was defined as abortion occurring with 4 doses of misoprostol or within 24 hours of administration of the first dose of misoprostol. The dose was not repeated after 4 tablets. Outcome measures included successful termination rate mean expulsion time and side affects of medication.
Results: Ninety percent aborted successfully. The mean dose of misoprostol required was 500 ug and the median induction to abortion interval after first misoprostol administration was 10.5 hours. A total of 7 required surgical evacuation under general anesthesia. Three percent had failure even after the 4 doses of misoprostol.
Conclusion: Misoprostol is a safe, effective and economical drug for mid trimester termination of pregnancy in a developing country like Pakistan. |
