AFTER REMOVAL OF PTERYGIUM ROLE OF MITOMYCIN AND CONJUNCTIVAL AUTOGRAFT

ABSTRACT

Objective: Purpose of this study is to find out appropriate surgical technique, which has lower rate of recurrences and minimum complications.

Design: Prespective Patients and Methods: A comparative study of two surgical treatment of pterygium removal has been carried out in Abbasi Shaheed Hospital and Karachi Medical and Dental College. The study period was 12 months starting from February 2003. A total no. of 102 patients, (130 eyes) with primary pterygium are included in this study. These patients were placed into group-I and group-II. Group I consist of 82 patients (114 eyes). In this group pterygium excision was done will bare sclera technique and mitomycin C eye drops were used post-operatively 2 times daily for 3 days. The group II consists of 20 patients (20 eyes). In these patients, the pterygium excision combined with conjunctival autograft. The follow up ranged 6-9 months. The mean follow-up time was upto 7.32 months.

Results: The pterygium recurred in 1 patient (1.36%) in group I, and in 2 patients (13.33%) in group II, complication observed in group I patients are irritability and lacrimation in 16 patients (21.9%) avascularized sclera in 12 patients (16.43%), conjunctival granuloma in 2 patients (2.73%) and superficial punctuate keratitis in 1 patients (1.36%). In group II patients conjunctival cyst formed in 2 patients (13.33%), conjunctival granuloma in 1 patient (6.66%) and wound gaping was observed in 1 patient (6.6%).

Conclusion: The study indicates that post-operative use of 0.02% mitomycin C eye drops twice daily for 3 days is a safe and effective treatment for prevention of recurrence of pterygium.
