MANAGEMENT OF ENDODONTICALLY FAILED
MANDIBULAR RIGHT FIRST MOLAR

ABSTRACT
Endodontic retreatment is a procedure performed on a tooth that has received prior attempted definitive treatment resulting in a condition requiring further endodontic intervention to achieve a successful result.1
Endodontic retreatment is indicated where there are technical deficiencies, procedural errors, pain, swelling or , a sinus tract, or when persistent symptoms are present. The overall reported success rate of endodontic retreatment varies from 60% to 98% and the success is higher if non-surgical techniques can be performed. Studies have shown that the success rate in Pakistan is 83% with only 17% failed cases, which required surgical intervention or extraction.2 Microorganisms are the main causative factors in patients reporting with periradicular disease in failed endodontic therapy. Despite the rise in cases with failure of root canal fillings retreatment procedure is still not commonly done in public sector hospitals as retreatment of failing endodontic therapy can range from simple procedures to very complex and time-consuming approaches that require the use of specialized equipment.
In this case report root canal retreatment of a 27 year old patient is done who presented with pain in tooth # 46 due to failed endodontic therapy. She had received initial root canal therapy almost 6 weeks earlier, which later developed symptoms. There was gross tooth structure loss with chronic infection. Endodontic retreatment was chosen as a conservative approach considering the young age of the patient with multiple carious and missing teeth.
